Jack Anderson Presents Poster at Academy Health Conference

Jack Anderson, a Project and Policy Coordinator with the UCSF Optimizing Aging Collaborative, worked on a study recently presented at the national Academy Health Conference. The study was a state-wide survey of 906 primary care providers (PCPs) in Nebraska which found that PCPs are highly active in engaging patients with chronic diseases in self-management techniques and linking them with community services. PCPs affiliated with larger health systems had more referrals and linkages than those in private practice.
