The Healthcare system is made up of many different settings and can be difficult to navigate. However, our faculty have expertise in providing care across the healthcare continuum including outpatient clinics, hospitals, home-based care, nursing facilities and hospice. In each setting our geriatricians work with a team of healthcare providers to maximize patients’ function and quality of life.

Bridges provides home-based palliative and transitional care consultations for UCSF adult patients of all ages.
We are a transitional care program for people of all ages who have a UCSF primary care physician in Family & Community Medicine or General Internal Medicine and need extra support at home after discharge from the UCSF Medical Center.
Geriatrics, Palliative and Extended Care is a coordinated system of care across the care continuum for older veterans at the San Francisco VA Medical Center. This system includes: Home-Based Primary Care, Outpatient Geriatrics Clinic, Outpatient Palliative Care Clinic, Inpatient Hospice Unit, Inpatient Acute Care for Elders (ACE) Unit, PACT Intensive Management Program for patients who need extra support at home, and a Community Living Center for long-term care.
The San Francisco General Hospital has two Acute Care for Elders (ACE) Units for hospitalized older patients as well as a Geriatrics Outpatient Clinic on Saturdays.
