Improving Advance Care Planning at ZSFG and UCSF

The PREPARE for Your Care program improved advance care planning among English and Spanish-speaking primary care patients at Zuckerberg San Francisco General Hospital and Trauma Center.

Using the PREPARE for Your Care program and automated messages in the electronic health record, advance care planning was improved across primary care at UCSF.
