About us:

Dementia Care Aware is a training and support program that empowers primary care providers and their teams to assess and address dementia. We provide online training on an initial 5-10-minute screening called the cognitive health assessment (CHA) and additional training on important topics and resources for the next steps in evaluation and care planning with patients. We also provide tools for implementing dementia detection and care in clinical practice as well as connecting patients and care partners to community resources.

What we offer:

The cornerstone of our program is a training on an annual screening tool for people 65 or older called the cognitive health assessment (CHA). The CHA is a dementia screening approach intended to be conducted in older adult patients whenever a cognitive screen is indicated. Click here for more information!

Dementia Care Aware also holds monthly webinars and releases monthly podcasts on dementia screening and care topics. Check out our upcoming webinars and podcasts on our website!

We also offer the Dementia Care Aware Warmline, a primary care team support and consultation service. This service is designed to provide decision-making consultation for clinicians and primary care teams in California around dementia screening, assessment, diagnosis, management, and care planning. Consultants can answer questions that arise during any part of dementia care Monday through Friday, 9 am - 5 pm PST at 1-800-933-1789 or through our online support form.
