Caregivers As Partners in Care Teams (CAP-CT)
About us:
Caregivers As Partners in Care Teams (CAP-CT) is a national training program that provides health care teams with the skills and confidence to include family caregivers in a patient's care journey. Through training and practical resources, health care teams can learn to identify, engage, and support caregivers, leading to better care for patients and their caregivers.
What we offer:
CAP-CT offers a library of courses, along with resources and tools. Courses cover essential communication, coordination, and system-wide approaches to improve patients’ health and well-being through engaging caregivers. Courses range from 15-60 minutes and can be taken as a self-paced online course or as a live training with an instructor. All courses offer free continuing education credit.
CAP-CT also provides tools such as the Caregiver Identification Tool, billing guides, and a curated list of resources that health care teams can use to connect caregivers to necessary support services.
