Age Friendly Inpatient and Outpatient Programs Win Awards for Value Improvement

Our Age Friendly Health System Program won two FY18 Value Improvement Awards from UCSF Health.  First, our Age Friendly Inpatient & Outpatient Program was one of two programs at UCSF awarded Best Partnership Across Initiatives.  Age Friendly leaders in Care at Home and Inpatient Geriatrics (Drs. Carla Perissinotto and Stephanie Rogers) worked closely together to coordinate care for patients upon discharge, ensuring a smooth transition home.  These teams also worked together to develop protocols to notify the care team when Care at Home patients were admitted to UCSF to ensure continuity of care.  In addition, the Age Friendly Inpatient team won the award for Best Utilization of Visual Management.  They incorporated excellent visualization of key metrics of success, including caregiver engagement, patient satisfaction, falls, time to the operating room, infection rates, length of stay and direct costs per case into their True North Board on 15 Long.  At each quarterly report out in FY18, Dr. Rogers brought the multiple Age Friendly initiatives, including delirium reduction, ACE and hip fracture co-management to life through creative, illustrative slides. Recently Dr. Rogers’ work was highlighted at the UCSF Health Strategy Retreat where she shared her team’s progress with the Chancellor, UCSF Health and School of Medicine leadership.  There will be a Value Improvement Reception on Thursday September 6th from 4:30-6pm at Mission Bay Oberndorf Auditorium where Mark Laret will recognize award winners.
