Bay Area Older Adult Community Resource of the Week – Clarity Pro

This week’s Geri Community Resource is: Clarity Pro (pro.planforclarity.org).

Clarity Pro (pro.planforclarity.org) is a new free tool from the UCSF-UC Law SF Medical-Legal Partnership for Seniors to help health care teams identify the needs of low-income older adult patients and caregivers and connect them to the right supports. These tools are especially applicable to older individuals at risk of institutional long term care and/or conservatorship.

Clarity Pro has four interactive care navigation tools to help:

  • Identify unmet legal and financial planning needs
  • Provide patient and caregiver education on the spectrum of LTSS/LTC and who pays
  • Address unmet social needs
  • Assess alternatives to conservatorship, and
  • Find legal help

Patient and caregiver-facing materials are available in English and Spanish for all 50 states, DC and Puerto Rico.

Clarity Pro also provides educational handouts and materials to help health care teams improve their knowledge and capabilities in addressing these needs. 

Clarity Pro is free thanks to the generous support of the AARP Foundation and was developed with previous funding from the National Institutes on Aging.
