Bay Area Older Adult Community Resource of the Week - Medical Legal Partnership for Seniors program
This week's "Bay Area Older Adult Community Resource of the week" to be featured is the Medical Legal Partnership for Seniors (MLPS) Program!
Yvonne Troya is the legal director and pioneer of this amazing organization, Medical Legal Partnership for Seniors (MLPS). She oversees the team of student lawyers that offer a multitude of services to geriatric patients. MLPS is currently being utilized by the inpatient and outpatient Geriatric department, VA Geriatric clinic(CLC), General Hospital, Institute on Aging, care support team here at UCSF. Attached to this email and also in our BOX shared file under Resources and Handouts, folder name is MLPS, is a brief slide summary of the program, a brochure, and the referral form.
Synopsis of coordination of care
- Provider would identify a need for advocacy for the patient
- Provider would discuss MLPS briefly with patient, and a referral form would be filled out by our staff and signed by patient and sent to MLPS
- MLPS meets with patients at clinic (if urgent), at home, or during Covid times on the phone and identifies solutions for the patients
- MLPS resolves the issues and if needed will refer to the proper entity
- MLPS then reports back to the providers providing outcomes and solutions obtained for the patients
Some of the many services MLPS can provide for the patients:
- Advance Directives, Durable power of Attorney, simple wills, trusts
- Housing advocacy, SSI advocacy, food stamps, conservatorship, caretaker advocacy
- Elder abuse advocacy, financial abuse
