Bay Area Older Adult Community Resource of the Week – UCSF Center for Geriatric Care (UCGC)

This week’s Geri Community Resource is: UCSF Center for Geriatric Care (UCGC).

UCSF Center for Geriatric Care (UCGC) provides personalized, expert care tailored to the unique needs of older adults by a specialized team of geriatric physicians, nurses, and social workers. Patients have access to on-site psychiatry, psychology, and phlebotomy services with physical therapy and imaging services nearby. In order to receive primary care at UCGC, a patient must be age 75 or older living in San Francisco or age 65 to 74 living in San Francisco with dementia. A patient can receive consultative visits to support ongoing care if they have a primary care provider at UCSF already. To get started, you can schedule your appointment here.
