Drs. James Harrison, Rebecca Sudore, Andrew Auerbach, Sachin Shah, Margaret Fang and colleagues Margaret Wheeler, MD and Sandra Oreper, MPH, published in the Journal of the American Geriatrics Society demonstrating that post-hospital automated calls are feasible and effective at reaching older adults. However, gaps in discharge communication ensuring that older adults are aware of their follow-up plan and appointments persist.
