Kudos for Outstanding Care Coordination

We received the following message from an inpatient case manager at UCSF Parnassus recognizing the excellent work of Grant Tominaga, our UCSF patient navigator at the San Francisco Campus for Jewish Home (SFCJL):

“I wanted to take a moment to share my sincere appreciation for the opportunity to work with Grant, your patient navigator at the Jewish Home last week. He was a tremendous help in ensuring that UCSF patients were seamlessly connected to appropriate outpatient services. The case we collaborated on was particularly challenging, and Grant was consistently open, responsive, and collaborative with the Jewish Home Case Management team. His professionalism and willingness to problem-solve together made a meaningful difference in navigating the complexities of the situation. It was truly impressive to see how UCSF prioritizes timely, thorough follow-up care for its patients. The coordination and commitment demonstrated throughout this process reflect a high standard of patient-centered care. Kudos to your wonderful team!!!”

Thank you, Grant, for your dedication and for representing UCSF and SFCJL so well through your collaborative, patient-centered work.
