New AFED Consult Note Template Aligns with CMS Age-Friendly Care Measures

With contributions from Anna Hines, NP, Christy Sedore, RN, the APEX AC3 Team, and other collaborators, a revised AFED Consult Note Template was published in APEX on March 30. The updated template aligns with the new CMS Age-Friendly Care quality measure in preparation for future reporting requirements.

It incorporates five key domains for patients aged 65 and older: eliciting patient healthcare goals, responsible medication management, frailty screening and intervention, patient vulnerability, and age-friendly care leadership.
