Rebecca Sudore and UCLA Collaborators Awarded PCORI Grant to Expand Advance Care Planning

Rebecca Sudore, MD and collaborators Drs. Neil Wenger and Anne Walling at UCLA have been awarded a PCORI implementation grant to expand advance care planning support for seriously ill primary care patients.

The three-year project will implement automated EHR messages about advance care planning and the PREPARE for Your Care program for primary care patients at UC Davis, UC Irvine, UC Riverside, Cedars-Sinai, Alameda Health System, the University of Minnesota Health System, and Washington University in St. Louis.
