Rebecca Sudore Publishes ACP Trial in Annals

Rebecca Sudore, MD, and colleagues published “Interventions to Improve Advance Care Planning Documentation in the Electronic Health Record” in Annals of Internal Medicine. The trial across 50 clinics showed that adding health navigator outreach to PREPAREforYourCare.org resources increased ACP documentation for seriously ill patients.
