UCSF Osher Center Welcomes Dr. Susan Schneider to Integrative Aging & Geriatric Medicine

The UCSF Osher Center is pleased to welcome Susan Schneider, MD, who will be joining Maki Nakazato, MD to offer Integrative Aging/Geriatric Medicine consultations. She and Dr. Nakazato provide whole-person, consultative care for older adults, partnering with patients and primary teams to support healthy aging, independence, and quality of life. 

Referrals for individual or group medical visits ("AGE SELF CARE") can be placed through the Osher Center APeX smart referral (ref39), for more information please visit our website.

Appropriate patient referrals include adults 65+ who have:

  • Interest in integrative, mind-body and whole-person approaches
  • Memory concerns or dementia 
  • Falls, balance, or mobility problems 
  • Functional decline or increasing need for assistance
  • Multiple medications or supplement questions 
  • Weight loss or nutrition concerns 
  • Chronic pain, sleep, or mood concerns 
  • Frequent hospitalizations or complex medical needs 
  • Advance care planning or goals-of-care discussions 
  • Serious illness, palliative, or end-of-life care needs
