Welcome back Jackie Buente!

We are delighted to announce that Jackie Buente, LCSW, ECDCS, has returned to the Division of Geriatrics as our new GUIDE Program & Operations Manager!

Jackie started today and will be working closely with Dr. Anna Chodos to implement a new program funded by the U.S. Department of Health & Human Services (DHHS) Centers for Medicare & Medicaid Services (CMMS).  The Guiding an Improved Dementia Experience (GUIDE) Model will test whether providing an alternative payment methodology for participating dementia care programs to deliver a package of care management and coordination, caregiver education and support, and GUIDE Respite Services to Medicare beneficiaries with dementia and their caregivers reduces expenditures while preserving or enhancing quality of care.  As the new Program & Operations Manager, Jackie will oversee all program implementation functions, including adherence to ambulatory care clinical, regulatory, and operational practices.

Most recently, Jackie was our Clinical Social Worker on the Dementia Care Aware (DCA) team. This state-led initiative, funded by the Department of Health Care Services (DHCS), aims to increase cognitive health screening in older adults. Jackie’s responsibilities included developing interdisciplinary team workflows, documentation standards, and policy and procedures to support primary care clinics.  She has also been instrumental in creating and executing the medical social work role within the interdisciplinary team to support the implementation and follow-up of the cognitive health assessments developed by Dr. Chodos and her team.  We are so excited to welcome Jackie back to the team in this new capacity!  Jackie will be stationed at the Mission Bay Office, cubicle 8115.  She can be reached at [email protected].  Please join us in welcoming her back to the team!
