Welcome Camie Kuo and Journey Meadows, Nurse Practitioners, to the Division of Geriatrics!

Please join us in welcoming Camie Kuo and Journey Meadows, Age-Friendly Emergency Department (AFED) Nurse Practitioners, to the Division of Geriatrics! 

The UCSF Emergency Department, in partnership with the Division of Geriatrics, operates an Age-Friendly Emergency Department Program (AFED).  AFED’s mission is to ensure a patient-valued older adult emergency department experience through high quality resource optimization, community relationships, diversity of engaged health professionals, respecting the patient desire to attain their desired level of independence and function.  Camie and Journey will be supporting the AFED consult service and working closely with a multi-disciplinary team of pharmacists, ED providers, geriatricians, social workers, community-based providers, and other interdisciplinary professionals and services to create an optimal discharge or ongoing plan of care for our patients.

Camie has a Master of Science degree in Family Nurse Practitioner from UCSF, School of Nursing, a Master of Arts in Experimental Psychology from UC San Diego, is certified as an AANP Family Nurse Practitioner and is a licensed NP and RN.  Camie comes to us with close to 16 years of direct patient care experience working in various healthcare settings, and 5 years of primary care experience serving the vulnerable Medicare-Medi-Cal patient population.  She is a UCSF-trained Family Nurse Practitioner committed to thoughtful patient-centric care and has worked in a variety of healthcare systems including: Alignment Healthcare in San Diego, Landmark Health in South San Francisco and with the Family Medical Group based in San Leandro.  She also worked as a Registered Nurse at Zuckerberg San Francisco General and worked at CMPC as a Nurse Practitioner.  Camie is fluent in Mandarin and already well-adept in the UC systems. 

Journey is a Board-Certified Adult-Gerontology Acute Care Nurse Practitioner with a Master of Science degree from UCSF.  She is the Associate Medical Director at LifeLong Medical Care, Over Sixty Health Center in Berkeley, where was also a Nurse Practitioner for 7 years and managed a panel of 300 medically and psycho-socially complex older adults across several interfaces of healthcare-SNF to home to clinic.  She also worked in the Emergency Department and Department of Adult Medicine at Highland Hospital, so she is also highly-experienced in working with multi-disciplinary teams in a fast-paced environment!.  Journey is a Center for Care Innovations, Innovations Scholar in 2017 and won an Excellence in Teaching and Clinical Leadership award in 2016.  Journey also led impactful quality improvement projects including: a Clinician Wellness/Burnout Assessment at LifeLong, and an Advanced Directive Initiative surveying the rate of completion of Advanced Care Planning in the Care at Home Program.
