Welcome Irina Kaplan!

Please welcome Irina Kaplan, RN, to UCSF Health Office of Population Health Care at Home.  Irina will serve as the new Care at Home Manager.  Throughout her career, Irina has worked in a variety of clinical settings, with the majority of her experience working with the vulnerable geriatric population.  Prior to her role at UCSF, she worked as a Care Transitions Program Manager at San Mateo Medical Center.  In this role she develop a transitions program for all at-risk community members in addition to leading the clinical team.  Her other professional experiences included working at the Health Plan of San Mateo, in addition to serving as the Director of Clinical Services for a Bay Area Home Health Program.  She has received multiple awards for outstanding performance, including the San Mateo County STARS award and was presented with the Clinician of the Year from Maxim Healthcare Services. 

Irina lives in Foster City with her husband, an 18-year old daughter and 9-year old son. She enjoys traveling and spending time outdoors with her family.  She is a foodie and love learning about new cultures through food.
