Improving Advance Care Planning Among Older Adults in Permanent Supportive Housing

This five-year National Institutes of Health R01 grant (2023-2028), co-directed with Drs. Margot Kushel and Margaret Handly, is funded to refine the PREPARE for Your Care program for formerly chronically homeless older adults living in 30 permanent supportive housing locations across the Bay Area. The study compares the effectiveness of facilitated group sessions using PREPARE movies and a facilitator guide versus one-on-one visits.
