Improving Care Planning and Deprescribing for Older Adults with Type 2 Diabetes

This five-year National Institutes of Health R01 grant (2020-2025), co-directed with Dr. Richard Grant of Kaiser Permanente Northern California, is funded to create and test the new PREPARE for Your Diabetes Care to help older adults talk to their medical providers about their diabetes medications and deprescribing to help prevent hypoglycemic events.
