This week’s Geri Community Resource is: Project Open Hand’s Wellness Program.
Project Open Hand’s Wellness Program provides nutritional support to eligible Medi-Cal residents of San Francisco and Alameda County with COPD, congestive heart failure, Type 2 diabetes, end-stage renal disease, HIV/AIDS, or who have had major surgery within 30 days of discharge. Referrals require an application completed and signed by a licensed medical provider to be considered for the program.
