Dementia Care Aware Selected as IHI Action Community Facilitator

Big news for the Dementia Care Aware team!

This March, we're proud to announce that Dementia Care Aware has been selected by the Institute for Healthcare Improvement (IHI) to serve as an Action Community Facilitator, supporting health systems across the country in becoming Age-Friendly, including the option to implement DCA's dementia screening and care pathway. This is a tremendous recognition of DCA's leadership in making dementia screening more routine.
