This investigation researched the benefits of a healthcare team guided approach to successful attendance at a formal evaluation at the UCSF Memory and Aging Center (MAC) versus the standard of care driven by the patient/caregiver dyad with their primary care provider guiding the referral. The direct referral benefits of the Age Friendly Emergency Department NPs to the MAC for patients with identified risk of cognitive impairment in coordination with the MAC greatly improved the success of attending an appointment and reducing equity access barriers for all patients receiving a timely evaluation when the benefits of novel therapies have their greatest benefit.
