Rebecca Sudore and PREPARE Team Creates PREPARE Toolkit

Dr. Rebecca Sudore and the PREPARE team have been collaborating with Dr. Kevin Valadares and the USI-GWEP team to create a Toolkit to walk organizations through how the patient-facing PREPARE resources are synergistic with the clinician-facing Respecting Choices program. Check out the innovative work the USI-GWEP program is doing in advance care planning and how they incorporated the PREPARE materials at their helpful video.
