Welcome back Gabriela Vanegas!

Please join us in welcoming Gabriela Vanegas to her new role as Dementia Care Aware Care Navigation Supervisor!

Gaby has been with the Geriatrics Division since June 2014 as Assistant Clinical Research Coordinator for The PREPARE Study.  She accepted a full-time position with the UCSF Division of Palliative Medicine in November 2020 as Health Care Navigator but kept a footprint with the PREPARE Team working on the PCORI study “Population-based comparison of evidence-based, patient-centered advance care planning interventions”.  As of 7/3, Gaby started her new role as Care Navigation Supervisor for the Dementia Care Aware team, reporting to Dr. Anna Chodos (Program Director).

Gaby graduated from Menlo College with a Bachelor of Science in Management and has showcased her dedication and commitment to improving patient care outcomes. Gaby brings a wealth of experience from her previous job roles in the healthcare industry.  As a Patient Enrollment Coordinator, she successfully scheduled appointments and ensured patients were reminded of their upcoming appointments.  As a Project Manager, she effectively managed goals and expectations to execute a clinical trial for an appointment reminder system tailored to diabetic patients.  Gaby's tenure at the UCSF Division of Geriatrics as an Assistant Clinical Research Coordinator allowed her to manage caseloads of patients, monitor their progress, and handle multiple petty cash transactions and check requests. She also conducted data entry in various databases, including REDcap, Access, and Excel.  As Health Care Navigator, Gaby conducted patient outreach to screen patients for palliative care or supportive care needs and help guide patients to appropriate services and helped to develop interventions and services in response to those screenings.  She also managed health care records and follow up visits and ensuring continuation of care and linkage to community resources.

As our new DCA Navigation Supervisor, Gaby will utilize her diverse skill set to lead our team and implement strategies that enhance the care provided to individuals with dementia.  She will work closely with the DCA San Francisco Health Network clinical team to execute care plans for patients with signs or symptoms of cognitive impairment. She will also refer patients to appropriate resources and will educate peers and clinical staff about services for older adults with cognitive and functional impairments.  Gaby will be stationed at the Mission Bay Office; 8th floor and can be reached at [email protected] (although her email is currently inactive, and she is out of the country until late August!)
