Welcome Dianne Montances!

Please welcome Dianne Montances, Geriatrics Inpatient Health Care Navigator!

Dianne will be primarily working with Dr. Stephanie Rogers and her care team to support the Inpatient Geriatric Service, which includes the Acute Care for Elders (ACE) Program and the Geriatric-Orthopedic Co-management service at Moffitt-Long Hospital. The mission of the Inpatient Geriatric Service is to provide the best inpatient care for hospitalized older adults with a focus on maintaining and improving physical and cognitive function.  In addition to her HCN role, Dianne will also serve as a Program Coordinator/Research Assistant to provide support to Dr. Rogers’ program/research needs.

Dianne graduated from San Jose State with a Bachelor’s degree in Kinesiology, as well as a minor in American Studies.  While in school, Dianne gained experience working with patients as an Orthopedic Physical Therapy Aide at Omega Sports Rehabilitation in Los Gatos.  She also worked as an Orthopedic Physical Therapy Aide at Physiotherapy Associates in San Francisco thereafter.  Her expertise includes: instructing patients in exercises, maintaining equipment, operating therapeutic modalities, and navigating patient resources.  She then went on to become an Orthosis Fitter at Joint Active Systems where she measured, delivered, and custom fitted splints on patients, instructed patients and health professionals on treatment protocols and collaborated with administration to verify insurance and follow-up with customers.  Most recently, Dianne worked as an Orthopedic, Sport, Hand, and Neuro Rehab Aide for over four years at Stanford Health Care.  With a total of 10+ years of customer service, direct patient care, and front desk management, Dianne’s new role as Health Care Navigator in IP Geriatrics will certainly bring a positive impact to the team and to our patients.  We are very excited to welcome Dianne on her first day (today).  She will be stationed at Laurel Heights, Suite 380 today and tomorrow (Friday).  Please welcome Dianne via her email at [email protected].
