Welcome Martha Lechlinski!

Martha Lechlinski started her new role as Health Care Navigator for Geriatrics Inpatient Service this morning, working closely with Dr. Stephanie Rogers and Megan Rathfon, NP to provide the best inpatient care for hospitalized older adults with a focus on maintaining and improving physical and cognitive function and ensuring safe transitions from hospital to home.  Martha has been at UCSF Health since 2011, serving in a number of roles and providing excellent patient care to UCSF patients.  Most recently, Martha worked as a Senior Licensed Vocational Nurse in the Women’s Health Primary Care clinic at Mt. Zion.  Martha has years of experience providing direct patient care including vaccinations, wound care, ear lavage, venipuncture, and medication administration.  She also educated patients on self-care, including nutrition, weight management, medication administration, hormone injections, blood pressure.  As a Senior LVN, Martha often trained new LVNs and MAs to provide the same level of service and care to our patients.  Prior to the Women’s Health Clinic, Martha also worked at the Multiple Sclerosis Center and the Department of Neurology at UCSF between 2011 and 2014.  Prior to coming to UCSF, Martha also gained invaluable experience working with the Geriatrics population when she served as the LVN/Charge Nurse at Emeritus Senior Living in Rancho Mirage.  There, she led nursing staff to provide care to 22 residents including wound care, medication administration, blood draws, injections, and IV care.  Martha brings her 7+ years of practice and experience as a clinician, supervisor, and patient educator to continue caring for our patients as our Health Care Navigator!  We are extremely excited that she has decided to join our team!  She will be stationed at the Moffitt Long Hospital, Room L1507.  Her phone # is 415-502-5010 and her email address is [email protected] if you need to get a hold of her.  If you happen to see her around, please make her feel welcome!
