Geriatrics-Orthopedics Co-management
About Us
All patients 70+ hospitalized for a hip fracture at Parnassus and Mount Zion will have their care managed in collaboration with a geriatrician. Teams meet daily to discuss each patient on interdisciplinary rounds. Examples of care provided include a comprehensive geriatrics assessment, focus on harm prevention (like falls and delirium), osteoporosis treatment, polypharmacy and out of hospital transitions.
How to Refer
No referral necessary. Patients in the hospital with a new hip fracture and those over 70 years-old admitted to orthopedics University service are automatically co-managed.
